If you have been denied coverage by your commercial prescription insurance, IncyteCARES for OPZELURA may be able to help
If you have commercial prescription insurance and have been denied coverage for OPZELURA after your doctor has submitted a prior authorization to your insurance plan, you may be eligible for the Commercial Access Program. Through the Commercial Access Program, you may be eligible to receive a limited quantity of OPZELURA at no cost.*
To be eligible for the Commercial Access Program, you must:
- Have commercial prescription drug insurance
- Have been denied coverage for OPZELURA by your commercial prescription drug insurance plan
- Have been prescribed OPZELURA for mild to moderate eczema
- Be considered clinically appropriate for OPZELURA based on the product prescribing information
How it works
If you are eligible for the Program, your doctor will need to complete and submit the Prescription and Enrollment Form for OPZELURA.
Once IncyteCARES for OPZELURA has received the completed form and you are eligible, you will be shipped one tube of OPZELURA from the designated Program pharmacy.
If you need a second tube of OPZELURA, your doctor must first appeal the initial coverage denial with your insurance plan.
